NACDD's Chronic Disease Data Modernization Action Incubator

A short-term, action focused opportunity aimed at establishing or strengthening multi-disciplinary state level leadership teams who can assess and help modernize chronic disease surveillance. Teams identify organizational assets, collaborate, innovate, and pilot data modernization projects with expert guidance and peer support. To keep informed on new opportunities, please complete the Data Modernization Accelerator Interest Form. 

PHII's Data Modernization Initiative Planning Toolkit

The toolkit offers guidance on making a value case for data modernization projects, including a worksheet to develop a value case for a new surveillance system, example value cases, and guidance for developing data modernization messaging.

ASTHO's Data Modernization Primer and Tactical Guides" with the description

The primer provides state and territorial health officials with a high-level understanding of the objective and significance of data modernization as well as the roles that they play in a successful data modernization initiative. The five tactical guides detail key strategies and tactics for implementing and maintaining data modernization initiatives within public health agencies.

Current chronic disease data modernization pilot projects

Health Trends Across Communities in Minnesota (HTAC)

A collaboration between healthcare systems, public health departments, health organizations, and health plans in Minnesota. HTAC uses summary reports from EHRs on chronic, behavioral, and mental health conditions. The information comes from 11 health systems participating in the Minnesota Electronic Health Record Consortium (MNEHRC). Information from the MNEHRC represents around 90% of Minnesotans’ healthcare, making HTAC a powerful tool to describe the health of many communities. This success story from Minnesota can be used as a case study to other state health departments on utilizing existing EHR data to improve chronic disease surveillance. The Multi-State EHR-Based Network for Disease Surveillance (MENDS)

MENDS is a national network that uses existing electronic health record (EHR) data—accessed through intermediaries like health information exchanges (HIEs)—to generate timely, modeled estimates of chronic disease prevalence and trends. Rather than creating new systems, MENDS leverages the existing health data ecosystem within public health and health care settings to produce a publicly available dashboard focused on conditions like diabetes, hypertension, and obesity.

The Michigan Department of Health and Human Services Chronic Disease Registry Linking Electronic Health Record Data (MDHH CHRONICLE)

CHRONICLE is a near–real-time data system that provides timely chronic disease data with more granular detail and enables patient-level data linkage across disparate data systems for a more comprehensive disease monitoring ecosystem. CHRONICLE uses admission, discharge, and transfer (ADT) data to identify patients with a hypertension or stroke diagnosis and monitor disease burden longitudinally.
